2) You agree to notify front desk of any medical conditions that the therapist should be aware of:
Spinal problems
High blood pressure
Injuries
Heart conditions
Migraines
Varicose vein

*Massage therapy is not a substitute for medical examination of diagnosis. I understand that the therapist does not prescribe medical treatments or pharmaceutical and does not perform any spinal adjustment. I am aware that if I have any serious medical diagnosis, I must provide a physician's written consent prior to services.

*I understand draping of the gentile area and gluteal cleavage will be used at all times during the session for all clients.

*The therapist must immediately end the Massage session if a client initiates any verbal or physical contact that is sexual in nature.
